Targeted Case Management

Targeted Case Management (TCM) is a collaborative assessment, planning, facilitation, and advocacy process for 
options and services to meet a person’s needs. The targeted case manager coordinates services and helps 
oversee a person centered plan. Staff will link individuals to needed services and monitor the results. 
Targeted Case Management is intended for people who require several mental health services and have 
difficulty accessing or sustaining services independently.
